Cafs is seeking dedicated and compassionate practitioners to join our Youth Services team as Targeted Care Packages (TCP) Key Workers, supporting children and young people with complex needs to achieve greater stability, connection and wellbeing.

Targeted Care Packages (TCPs) provide flexible, individualised supports to children and young people who are in, or at risk of entering, residential care. Working closely with children, families, carers, Child Protection and community services, TCPs help create stable, nurturing and sustainable care arrangements that improve long-term outcomes.
As a Targeted Care Packages Key Worker, you will:
The SW4 role is ideal for practitioners who enjoy direct case management and working collaboratively within a multidisciplinary team to support children and young people to thrive.
The SW5 role is suited to experienced practitioners who can manage more complex cases, provide guidance to colleagues and contribute to high-quality practice across the program.
