About the Role
The Out of Hospital Care (OHC) service incorporates the ComPacks, Safe and Supported at Home (SASH) and End of Life (EoL) programs. The OHC service supports hospital discharges and prevents avoidable admissions by providing short and medium-term packages of non-clinical care. The Care Coordinator role provides support to the OHC Team, managing client enquiries and completing various administrative tasks supporting ongoing client case management, ensuring the smooth functioning of the service.
This position will be based across Concord Repatriation General Hospital, Royal Prince Alfred Hospital and Canterbury Hospital.
