An exciting opportunity exists for a Clinical Nurse to join our sub-acute Transition Care Service in Walkerville on a full time basis. You will provide clinical leadership and expert nursing care to patients with complex and ongoing healthcare needs. Working as part of a multidisciplinary team, you will play a key role in supporting clinical excellence and contributing to positive patient outcomes in a dynamic and rewarding healthcare environment.Your opportunity Coordinate patient care and support smooth transitions across care settings. Delivery high‑quality, evidence‑based nursing care. Supporting governance, risk management, and service coordination. Contributing to operational processes and system improvement. Promoting consistent, safe, and effective clinical practice across programs. Participate in quality improvement, service development, and multidisciplinary collaboration. Provide clinical leadership and act as a resource for nursing teams. About you Registered Nurse with current unrestricted AHPRA registration. Minimum 3 years post registration experience. Experience or interest in governance, service coordination, or operational management. Advanced clinical decision making and critical analysis skills. Strong organisational and leadership capability. Ability to manage rosters, budgets and resources. Commitment to patient‑centred care and continuous improvement. Hold current First Aid and CPR certificates. Ability to participate in a rotating roster. Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain). If successful, willing and able to provide proof of up-to-date vaccinations.
