Case Manager

As a Case Manager, you will provide person-centred case management and coordination support to NDIS participants & their support workers. You will work closely with participants, families, support workers, allied health professionals and community services to ensure support is well-coordinated, goal-focused and aligned with individual needs.

This role is suited to someone passionate about improving outcomes for people with disabilities and enjoys building strong relationships, coordinating services and supporting participants to achieve greater independence and wellbeing.

Key Responsibilities

• Manage an allocated caseload of NDIS participants.
• Conduct participant assessments and identify individual goals, needs and support requirements.
• Develop, implement and review person-centred support plans and risk assessments.
• Coordinate services with internal teams, external providers and allied health professionals.
• Build positive relationships with participants, families, carers and stakeholders.
• Monitor participant progress and review supports to ensure outcomes are achieved.
• Support participants to exercise choice, control and independence.
• Maintain accurate case notes, reports and participant documentation.
• Identify risks, respond to incidents and support safeguarding practices.
• Contribute to service improvement and quality outcomes.

Pay: $40.00 – $45.00 per hour

Education:

• Bachelor Degree (Preferred)
