About the Role
We are seeking an experienced and compassionate Assistant Coordinator - Home Care to join our Home and Community Services team. You will be working closely with the Care Coordinator and assisting in the development of care plans for clients based on their individual needs and help to facilitate the delivery of care and services for clients in home care.
Key Responsibilities
Assist the Care and Services Coordinator to develop care plans and provide the highest quality of client care and support
Cover weekend shifts on a roster basis and working effectively under minimal supervision at these times
Assist in facilitation of appropriate and agreed care and services within client budget constraints
Maintain staff rosters and ensure client coverage after hours when required, including administration and approval of staff leave in the absence of Care and Services Coordinators
Monitor staffing levels and ensure staff are allocated according to client care needs
Communicate effectively with staff, clients, families and other members of the community
This position includes a salary sacrificed vehicle as per OLOC's Motor Vehicle Policy
About You
Minimum 2 years’ experience working in an Assistant Coordinator - Home Care (Aged Care) position within an aged care environment
Current NSW C Class Drivers Licence and willing to travel if required
Solid understanding of My Aged Care systems including the Support at Home model
Strong problem-solving skills and be able to multitask to meet deadlines
Be highly organised with excellent communication skills and a sense of urgency
An understanding of relevant aged and community care sector legislation, standards and guidelines is desirable
Mandatory Qualifications
Relevant tertiary qualification in Aged Care
