As a Program Manager, you will:

• Build and maintain strong referral networks (GPs, hospitals, allied health, support coordinators and community organisations)

• Identify and pursue new referral opportunities within the local community

• Support growth targets for Support at Home and NDIS services

• Conduct client intake, home visits and assessments

• Develop person-centered care plans aligned with funding guidelines

• Coordinate and monitor service delivery to ensure quality and continuity

• Monitor client budgets and ensure services remain within funding guidelines

• Act as the key contact for clients and families

You will play a key role in strengthening Holicare's community presence while ensuring high-quality, compliant and person-centered service delivery.

About You

• Experience in aged care, disability or community services

• Strong knowledge of Support at Home and/or NDIS funding models

• Strong case planning and case management skills

• Confidence in building professional networks and promoting services

• Excellent communication and stakeholder engagement skills

• Ability to manage KPIs and competing priorities

• Current driver's license and willingness to travel locally

• Experience with systems (e.g. Nightingale, EziPlan, Share Point)

• Understanding of quality and safeguarding frameworks

Qualifications in Community Services, Nursing, Social Work Case Management, or related fields are highly regarded
