The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Are you an experienced Social Worker passionate about making a meaningful difference in the lives of children and young people? An exciting opportunity exists to join the Out-Of-Home Care Health Pathway Program (OOHC HPP), where you will play a pivotal role in supporting the health, wellbeing and recovery of infants, children and young people living in Out-Of-Home Care.
As a Social Worker Level 3, you will deliver specialist trauma-informed assessment, intervention and consultation services for children and young people aged 0–18 years. Working within a multidisciplinary team, you will contribute to comprehensive health assessments and ongoing health management to promote, protect and maintain the physical, developmental, psychosocial, sexual and mental health of children and young people in care.
In this role, you will undertake comprehensive psychosocial assessments and contribute to the development, implementation and evaluation of Health Management Plans. You will provide targeted therapeutic and psychosocial interventions that address the impacts of trauma, adversity and disrupted attachments, ensuring care is responsive to each child’s developmental needs, placement circumstances and individual goals.
You will also provide specialist advice and consultation to carers, caseworkers and health professionals, supporting the delivery of coordinated and integrated care. Through education, group facilitation and capacity-building activities, you will help strengthen the skills and knowledge of those working with children and young people in Out-Of-Home Care, ultimately contributing to improved health and wellbeing outcomes.
