Physiotherapist – Aged Care & NDIS

About the role

As a Physiotherapist with Social Club Ability Services, you will work with clients across Aged Care and NDIS, providing assessments, treatment and ongoing physiotherapy support based on each client's individual needs and goals. You will deliver physiotherapy services within clients' homes and community settings, supporting them to maintain their mobility, independence, safety and quality of life.

Key responsibilities

• Conducting comprehensive physiotherapy and mobility assessments
• Developing individualised treatment and rehabilitation programs
• Providing strength, balance, mobility and falls-prevention interventions
• Supporting clients with pain management and functional mobility
• Assessing gait and recommending appropriate mobility strategies
• Providing recommendations for mobility aids and assistive equipment where appropriate
• Preparing clinical reports, progress notes and recommendations
• Liaising with care coordinators, support coordinators, families and other health professionals
• Supporting clients to maintain independence and safely participate in everyday activities

About you

• A recognised qualification in Physiotherapy
• Current AHPRA registration
• Current Australian driver's licence and access to a reliable vehicle
• Current National Police Check
• NDIS Worker Screening Check, or willingness to obtain one
• Strong communication and interpersonal skills
• Excellent clinical documentation and time-management skills
• A compassionate, respectful and person-centred approach
• Experience working within aged care, community health or the NDIS is highly regarded; however, physiotherapists looking to develop their experience in these areas are also encouraged to apply
