About the Opportunity
As a Social Worker you will provide quality trauma informed, person centred and culturally responsive clinical Social Work services to support patients/families/carers in their emotional response to the impact of illness and/or to facilitate transfer of care from hospital to community based services to support optimal health outcomes.

About You
We are looking for someone who has:
Demonstrated experience, preferably within a clinical service setting in providing psychosocial assessment and Social Work responses such as crisis and brief intervention, case planning/coordination, liaison, counselling, advocacy, and service referral.
Applicants must have current work rights in Australia or be a permanent resident or citizen. Unfortunately, we cannot consider applicants who do not meet this requirement.
