About the Opportunity

As a Social Worker you will provide quality trauma informed, person centred and culturally responsive clinical Social Work services to support patients/families/carers in their emotional response to the impact of illness and/or to facilitate transfer of care from hospital to community based services to support optimal health outcomes.

In this role you will:

• Providing quality Social Work evidence based practice within an environment of competing complexity, demands and priorities.
• Maintaining a high volume of diverse and complex casework and service demand.
• Provide Social Work clinical practices including psychosocial assessment, crisis and brief intervention, case planning, liaison, counselling, advocacy, and service referral.
• Enhancement of the Social Work role in an multi-disciplinary service setting.

About You

We are looking for someone who has:

• Tertiary qualifications in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW).
• Knowledge and understanding of the psychosocial issues that arise for patients and their families/carers when experiencing chronic/acute illness and trauma.

• Demonstrated experience, preferably within a clinical service setting in providing psychosocial assessment and Social Work responses such as crisis and brief intervention, case planning/coordination, liaison, counselling, advocacy, and service referral.
