One Doorway Job Description 2x Registered Nurse (Disability and Rehabilitation) Location: Narre Warren, Victoria Employment Type: Full-Time About the Role Logos Care Pty Ltd (Trading as One Doorway) is seeking a compassionate, dedicated and experienced 2x Registered Nurse (Disability and Rehabilitation) to join our growing multidisciplinary team.

The successful candidate will provide comprehensive nursing care and clinical support to participants living with physical, intellectual, neurological and psychosocial disabilities across community and Supported Independent Living (SIL) settings. You will work collaboratively with participants, families, allied health professionals and support workers to deliver person-centred care that promotes independence, wellbeing and quality of life. Key Responsibilities Deliver safe, evidence-based nursing care in accordance with AHPRA standards and the Nursing and Midwifery Board of Australia (NMBA) guidelines.

Conduct comprehensive health and nursing assessments. Develop, implement and evaluate individualised care plans. Administer medications and monitor therapeutic outcomes.

Perform clinical procedures including wound care, catheter care, PEG management, diabetes management, continence care and medication management. Monitor participants with complex health conditions and escalate concerns appropriately. Support participants during rehabilitation and recovery to maximise independence and functional capacity.

Educate participants, families and support staff on health management and clinical care. Maintain accurate clinical documentation, progress notes and incident reports. Participate in multidisciplinary case conferences and care planning meetings.

Ensure compliance with the NDIS Practice Standards, organisational policies and relevant legislation. Supervise, mentor and provide clinical guidance to Disability Support Workers and other care staff. Promote participant dignity, choice, rights and person-centred practice at all times.

Participate in quality improvement activities and ongoing professional development. Essential Requirements Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse. Minimum one (1) year of nursing experience.

Experience in disability services, rehabilitation, community nursing or aged care. Excellent clinical assessment and decision-making skills. Strong communication and interpersonal abilities.

Ability to work independently and as part of a multidisciplinary team. Sound understanding of the NDIS Practice Standards and person-centred care principles. Current National Police Check.

NDIS Worker Screening Check (or willingness to obtain). Current Working with Children Check (where applicable).

Current First Aid and CPR Certificate. Current unrestricted Australian Driver's Licence (or willingness to obtain).
