Aboriginal Community Services (ACS) are a non-profit provider of residential aged care and home and community services. We provide aged care services to our Elders / Clients across South Australia in metro and regional locations.
About the role
The major objective of this role is to provide complex case management and support for the clients to deliver their service plan. This will require working closely with the Team Leader, Client Services Coordinators (CHSP), Community Service Workers (CSWs); and brokered agencies.
Responsibilities:
Intake and assessment of new 'SUPPORT AT HOME' clients.
Provision of Case Management for complex care needs.
Develop, implement, and review client service/care plans within timeframes as per legislation, ACS policies and procedures.
Develop, implement, and review client budgets within timeframes as per legislation, ACS policies and procedures.
Understanding of chronic health and co-morbidities and monitoring of client health conditions.
Undertake annual review at minimum of client care needs, complete required documentation and processes within timeframes as per legislation, ACS policies and procedures.
Connecting clients with external agencies/stakeholders to provide additional supports that are outside of the funding guidelines.
Identify business development opportunities to support both organisational growth and continuous improvement activities.
Manage client services in accordance with SaH guidelines and the legislation to promote and achieve positive client outcomes.
Assist with referrals from Regional Assessment Services and ACAT in a timely and coordinated manner.
Develop strong collaborative relationship with other ACS staff / programs and external agencies to deliver seamless and integrated client services. Actively participate in a Duty Roster.
Incidents (SIRS Reporting), feedback, complaints and improvements reporting to be created in an accurate and timely manner and in accordance with legislation, ACS policies and procedures.
Oversight of contractors engaging with consumers.
Monitor communications from Community Support Workers and Agency staff to ensure timely follow up of consumer related incidents, feedback, complaint.
About you:
To be successful:
You will have achieved a tertiary qualification in community services, disability services or another related field OR carry 3+ years of previous relevant experience in a HCP/SaH Coordinator position.
You will possess excellent communication and organisational skills.
You have the knowledge and understanding of service and assessment models that support older people to live independently in the community.
You will also have a natural ability to actively engage clients in meaningful goal setting and care planning activities.
Previous experience working with Aboriginal people be highly regarded but not essential.
You have a current national police check or are willing to obtain one.
You will possess a current unrestricted Driver's License.
