What we're looking for

You're an experienced professional in aged care (perhaps a Home Care Package Coordinator, Care Partner, Enrolled Nurse, or CHSP Coordinator) ready to take the next step in your career.

You bring:

• Certificate IV in Community Services (or equivalent) and a current First Aid
• Proven experience in case management, service coordination, and care planning
• Strong understanding of SAH budgets and community care compliance
• Excellent communication, decision-making, and time management skills
• Confidence using digital tools (Word, Excel, calendars)

The role

• Conduct strengths-based assessments and create personalised care plans aligned with client needs and contractual obligations
• Provide person-centred case management and coordinate services for clients and carers with moderate to complex needs
• Manage budgets and ensure effective resource use
• Collaborate with health professionals and community organisations
• Maintain accurate records and meet reporting requirements
• Adapt to challenges with resilience and a solutions-focused mindset
