About the Role

Youth on Track Caseworker – 38hrs p/w Orange NSW

This is an identified position. Applicants must be of Aboriginal and/or Torres Strait Islander descent. The applicant’s race is a genuine occupational qualification and is authorised under Section 14 of the Anti-Discrimination Act 1977 (NSW).

Youth on Track is an early intervention program supporting young people aged 10–17 years who are at risk of becoming involved in the criminal justice system.

As a Youth Support Case Worker, you will work directly with young people and their families to provide culturally appropriate, strengths-based and holistic support. You will assist young people to address the issues contributing to antisocial and/or offending behaviour while building on their strengths, prosocial behaviours and connection to culture, family and community.

Supports are tailored to individual needs and may include case management, service coordination, referrals, mentoring, cultural activities, recreation, evidence-based programs and individual or group-based support.

Key Responsibilities

·         Work in partnership with young people, families, community and service providers to identify concerns and develop appropriate supports

·         Provide culturally safe, client-centred and strengths-based case management

·         Support young people and families to identify their own goals, concerns and solutions

·         Coordinate access to relevant services and maintain clear referral pathways

·         Facilitate access to cultural, social, educational, recreational and community-based supports

·         Build strong relationships with local service providers and community networks

·         Support young people and their families to achieve positive and sustainable change

·         Maintain accurate and timely case notes, case plans and client records

·         Promote young people’s connection to culture, family and community

About You

To be successful in this role, you will ideally have:

·         Experience working with young people and adolescents with complex needs and their families

·         Experience working within a case management framework

·         Experience working collaboratively with a broad network of services and maintaining effective referral pathways

·         Strong communication, relationship-building and advocacy skills

·         An understanding of the importance of culturally appropriate and trauma-informed service delivery

·         The ability to work independently while contributing positively to a team
