About the role
Coordinate care that makes a real difference.
At Leora Healthcare, care coordination is more than filling shifts. It’s about building trust, solving problems and making sure every client receives reliable, high-quality support.
About Leora Healthcare
Leora Healthcare has been caring for the community since 2008. We provide disability and aged care services designed to help people live with dignity, independence, and choice. Our vision is to be the most trusted care provider, and our mission is to empower people.
Our values - Clients First, Masterful Simplicity, Unleash Potential and Do the Right Thing - shape how we work with clients, alliances and each other. We are growing our presence in Melbourne and are looking for someone who can combine commercial drive with genuine care for people.
As a Care Coordinator, you’ll be the engine behind outstanding care delivery. You’ll take ownership of client services, support-worker coordination and day-to-day service outcomes.
We’re looking for someone with experience in aged care or the Support at Home (SAH) program who understands the importance of delivering services that align with client needs, budgets and care plans.
Coordinate client bookings and service requests accurately and efficiently.
Build rosters that meet client needs, budgets and SCHADS requirements.
Match clients with suitable support workers based on skills, preferences and availability.
Keep clients, families, support workers and internal teams informed.
Respond to roster changes, incidents and complaints with calm confidence.
Maintain accurate records and follow through until issues are resolved.
Build trusted relationships that support positive client outcomes.
You don’t just tick boxes—you take ownership and make things happen.
You’ll bring:
Experience in aged care, including exposure to the Support at Home program or home-care service delivery.
A relevant qualification in Health, Aged Care, Disability or Community Services.
Strong communication and relationship-building skills.
The ability to solve problems and make sound decisions under pressure.
Excellent organisation and attention to detail.
Confidence managing competing priorities in a fast-moving environment.
Knowledge of care plans, client budgets and the SCHADS Award will be highly regarded.
Join an award-winning, purpose-driven workplace.
Enjoy hybrid flexibility and genuine wellbeing benefits.
Access mentoring, professional development and career pathways.
Be recognised through team celebrations and rewards.
Make a meaningful difference in the lives of clients and their families.
If you have aged care or Support at Home experience and are ready to help deliver care people can rely on, we’d love to hear from you.
Apply now and help us build a future where exceptional care feels effortless.
