About the role

As a Care Partner, you'll manage a portfolio of Home Care Package clients, building trusted relationships and empowering older Australians to remain independent, safe and connected to their community.

Essential 

• Diploma qualification in a relevant field or higher (e.g. Diploma, Advanced Diploma, Bachelor's degree or above)
• Experience as a Care Partner or Case Manager within Support at Home, Home Care Packages
• Excellent communication and computer skills
• Understanding of aged care reforms and quality standards
• Ability to work autonomously and manage competing demands
• Commitment to customer service, cultural diversity, and workplace safety
• Willingness to travel as required and a current Victorian driver’s licence
• Satisfactory National Police Check

Desirable

• Knowledge of Carer Recognition Acts and relevant service frameworks (e.g., HCP, CHSP)
• Experience working with people from diverse backgrounds
