About the Position: The Community Support Worker plays a central role in coordinating and delivering high-quality support within our home care service.
You will manage client care plans, ensure regulatory compliance, assist frontline staff, and advocate for clients’ needs, drawing on strong care coordination, resource management and community engagement skills. This position is vital to maintaining the personalised, person-centred service that allows our diverse clients to achieve greater independence and wellbeing. Key Responsibilities:
Coordinate and manage individual client care plans in line with Home Care Package, Support at Home and NDIS requirements Support and guide frontline care staff to deliver consistent, high-quality services Advocate for clients’ needs and preferences while ensuring cultural, emotional and practical requirements are met
Monitor regulatory compliance, documentation and quality standards Engage with clients, families and community stakeholders to build effective support networks Respond to and manage crisis or complex situations with sound judgement
Contribute to continuous improvement of service delivery and client outcomes Requirements: Australian citizens and permanent residents are encouraged to apply
Diploma of Community Services Minimum 2 years’ relevant experience Strong care coordination, communication, leadership and crisis-management skills
Ability to work effectively with diverse clients and multicultural communities Sound understanding of community services, home care and NDIS frameworks Excellent interpersonal skills, empathy, professionalism and organisational ability
