About the Role
We are seeking a compassionate and experienced Registered Nurse to join our Melbourne team on a full-time basis.
This role combines clinical nursing expertise with case management and care coordination, supporting older people receiving services through the Support at Home program. You will work closely with clients, families and the multidisciplinary team to deliver high-quality, person-centred care in accordance with individual needs and industry standards.
Key Responsibilities
Including but not limited to:
Conduct clinical and comprehensive assessments to identify client needs and support ongoing care planning
Develop, review and monitor individualised care plans
Coordinate and support the delivery of client services in collaboration with internal teams
Liaise with clients, families and the multidisciplinary team to deliver person-centred care
Complete accurate clinical documentation, reassessments and care records
Ensure services are delivered in accordance with aged care legislation, clinical standards and organisational policies
About You
To be successful in this role, you will have:
Current AHPRA registration as a Registered Nurse
Demonstrated experience (ideally 2–3 years) as a Registered Nurse or in a similar clinical/case management role within the aged care sector, with experience in Support at Home and/or Home Care Packages (HCP) highly preferred
Strong clinical assessment and care planning skills
Excellent communication, interpersonal and organisational skills
A compassionate, client-focused approach with a genuine commitment to improving the wellbeing of older Australians
Ability to work independently while collaborating within a multidisciplinary team
Current Australian driver's licence and willingness to travel within the community
Fluency in English and Chinese is essential. Proficiency in both Mandarin and Cantonese will be highly regarded.
