What you will be doing

The Clinical Care Partner is the designated care manager under the Support at Home Program, responsible for delivering person-centred, culturally safe and rights-based care management that enables older people to live safely, independently and with dignity at home. The role assesses and monitors participant needs, develops and reviews care plans, coordinates services and multidisciplinary supports, and responds to changing circumstances, with a strong focus on wellness, reablement, choice, control, supported decision-making and risk management. Clinical activities are undertaken within the employee’s professional scope, competence and PRONIA’s clinical governance requirements, ensuring compliance with relevant aged care legislation, standards and organisational policies.

Key Responsibilities

• Develop, implement and review person-centred care plans and coordinate services that reflect participant needs, goals, preferences and cultural requirements, while promoting independence, wellness and reablement.
• Apply clinical and professional expertise within scope of practice to identify and manage risks, complex needs and changes in circumstances, ensuring appropriate escalation, referral and multidisciplinary support.
• Coordinate relevant Support at Home pathways and services, including restorative care, end-of-life care and AT-HM, while ensuring services and expenditure align with participant needs, approved funding and program requirements.
• Maintain strong relationships with participants, families, carers and providers; monitor outcomes and service delivery; and promote choice, autonomy, supported decision-making, education and self-management.
• Maintain accurate and timely records and ensure care management complies with legislation, Aged Care Quality Standards, Support at Home requirements, PRONIA policies, clinical governance and reporting obligations, including incidents and SIRS.
• Support participant feedback and complaints, respond appropriately to concerns and safeguarding matters, and maintain privacy, confidentiality, information security and accurate use of PRONIA systems.

Role Requirements

• Relevant experience in aged care, community services, health or a related sector.
• Ability to work independently while contributing effectively to a multidisciplinary team.
• Ability to work within professional boundaries, organisational policies and relevant legislation.
• Ability to use electronic client management and documentation systems.
• Current driver’s licence and ability to travel within the community, where required.

Mandatory Requirements - Clinical Care Partner

• Relevant tertiary qualification in Nursing or Allied Health.
• Current professional registration or accreditation.

The ideal candidate will have:

• Strong knowledge of the Australian aged care sector and the Support at Home Program.
• Understanding of the Aged Care Act, Strengthened Aged Care Quality Standards, Statement of Rights and Aged Care Code of Conduct.
• A strong commitment to person-centred, rights-based and culturally responsive practice.
• Excellent communication and interpersonal skills, with the ability to build trusted relationships.
• Sound professional judgement, discretion and a high level of integrity.
• A proactive, adaptable and solutions-focused approach.
• Strong digital literacy and written communication skills.
• Commitment to continuous learning, reflective practice and quality improvement.
• Experience working with culturally and linguistically diverse communities.
• Ability to speak and understand Greek language in highly desirable, to communicate with Greek-speaking participants, families and carers.
• Competence in interpreting clinical guidance and escalating appropriately.

What we offer

• Full-time, permanent role based in Brunswick or Oakleigh – (some travel to alternate office as needed).
• Remuneration is in accordance with the SCHADS or Nurses or Allied Health Award dependent on qualifications and experience.
• Salary Packaging up to $18,500 per annum.
