About the role
We are seeking a Registered Nurse to work permanently or casually as an RN Service Coordinator, to join our friendly team in Sydney. The role involves liaising with case managers, medical teams, families and the clients themselves to ensure the provision of high quality, at home care.
Complete Care Team has a holistic and wellness model of attendant care for people with high support needs, providing a quality home based person-centred service.
We specialise in providing support for clients with a brain injury or spinal injury and are looking for a service coordinator with industry relevant qualifications or demonstrable experience. Understanding of and experience in spinal, brain injury, wound management, bowel and catheter care, quadriplegic and disability support is preferred.
Must be willing to travel across Sydney and to other NSW locations as required.
Key Responsibilities:
Regular evaluation of client’s services
Developing and updating client Individual Support Plans
Liaising with Case Managers and other professionals involved in a client’s care
Support with day-to-day management of client’s needs
Compliance with WH&S legislation and company policy and procedure
Support with clinical care and clinical visits for clients
Provide training and clinical support to care workers

Essential Criteria
Qualifications and current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
Demonstrated Leadership skills and attitudes
Demonstrated ability to support and mentor attendant care workers and nursing staff. This includes assessing the competence of attendant care workers and nurses including the reporting of progress / issues and delivering timely and accurate feedback.
Proven clinical expertise and knowledge in the assessment, treatment and evaluation of clients, using evidence-based practice, relevant to home-based care
Demonstrated effective written and verbal communication skills
Demonstrated ability to consult, negotiate and collaborate with internal and external key stakeholders, clients and colleagues within the multidisciplinary environment to cultivate productive working relationships and to support best health outcomes for clients
A demonstrated ability to operate autonomously and apply sound judgement when making clinical management decisions. Proven commitment and ability in the development of a learning, teaching and research orientated work environment
Demonstrated commitment to WH&S and the positive client experience and behaviour consistent with CCT values.
Driver’s license and access to a vehicle
