Female Disability Support Worker – Mental Health & Alcohol Dependency Experience

Grace Quality Care Pty Ltd is seeking a vibrant, compassionate and confident female Disability Support Worker to support a middle-aged participant located in the Diamond Creek area.

We are looking for someone who is energetic, engaging, patient and reliable, while also having the maturity and professional experience required to support a participant with mental health challenges and alcohol dependency.

About the Role

The successful support worker will provide person-centred support aimed at promoting the participant’s independence, wellbeing, safety and community participation.

Duties may include:

• Providing emotional and practical support
• Supporting the participant with daily living activities and routines
• Community access, appointments, shopping and social activities
• Encouraging positive routines and meaningful engagement
• Supporting the participant to work towards their NDIS goals
• Providing appropriate support around mental health challenges
• Recognising changes in behaviour, mood or wellbeing and responding appropriately
• Maintaining professional boundaries when supporting a participant experiencing alcohol dependency
• Monitoring and reporting identified risks or concerns
• Completing accurate progress notes and incident documentation where required
• Communicating effectively with the participant and broader support team

• Able to establish rapport while maintaining strong professional boundaries
• Reliable, punctual and consistent
• Experienced in supporting people with mental health conditions
• Experienced in working with clients experiencing alcohol dependency or substance-use related challenges
• Able to recognise risks and escalate concerns appropriately
• Focused on participant choice, dignity and independence

Essential Requirements

Applicants should have:

• Previous disability, mental health, community services or similar support experience
• Demonstrated experience supporting clients with mental health and alcohol dependency
• Current NDIS Worker Screening Check
• Working with Children Check, where applicable
• Current First Aid and CPR
• Valid Australian driver's licence
• Reliable and insured vehicle
• Appropriate qualifications in Disability, Mental Health, Community Services, Individual Support or a related field are highly regarded
