The Role – Full-time | Home Care Service Advisor | Merrylands

This role involves regular travel. Flexibility to work across Sydney locations is essential.

Your role as a Home Care Service Advisor is to deliver client-centered services that promote and encompass an excellent standard of care through assessment, advice, and application across all Southern Cross Care (SCC) Home Support Programs

This role is pivotal in ensuring that clients' physical, clinical, emotional, and spiritual needs are met holistically. Additionally, the role involves maintaining comprehensive client and business documentation, including incidents management, care notes, and plans, to support high-quality care and compliance.

 The Home Care Service Advisor will work closely with Home Care Employees, the Quality team, and Allied Health professionals to ensure a coordinated and comprehensive approach to client care. This collaboration is essential for delivering high-quality, holistic care and achieving the best possible outcomes for SCC clients. 

What You’ll Be Doing

• Conduct SCC client assessments, care planning and reviews to ensure high-quality, person-centred home care services.
• Coordinate service delivery, referrals and stakeholder communication to support positive client outcomes and independence.
• Manage client funding, budgets and resources in line with Home Care Program requirements and organisational guidelines.
• Maintain accurate documentation, ensure compliance with aged care standards, and support quality improvement initiatives.
• Build strong relationships with clients, families and team members while delivering exceptional customer service and professional support.

Essential

• Certificate IV in Aged Care, Home Care, Management or other related area.
• Demonstrated experience in a similar role as Home Care Service Advisor.
• Demonstrated experience and understanding of Home Care Programs
• Understanding of current issues for clients and their networks in the Home Support service environment.
• Lead, escalate, manage and be accountable for the service delivery of Home Care Services across the service areas/Home Care portfolio.
• To undertake care/service delivery in the event of a Home Care Employee is not available, and or where brokerage cannot be coordinated/delivered. 

Desirable

• Rostering and administration.
• Demonstrated business administration skills in the area of documentation, record keeping, reporting, budgeting and quality auditing.
• Ability to problem solve and think on your feet, ensuring a high standard of services are delivered in SCC clients.
• Empathy and sensitivity to the challenges of delivering high quality services to the aged in their own communities and homes
