The opportunity
This is a part-time position (minimum 3 days) which includes driving and conducting face-to-face assessments in the client's home.
As a clinical Aged Care Needs Assessor - your primary objective is to conduct comprehensive, holistic assessments of older individuals to determine their eligibility for government – subsidised aged care services.
Responsibilities:
Complete face to face assessments to determine older Australians’ eligibility for support within the Clinical Aged Care Needs Assessment space.
Maintain a strong working knowledge of the My Aged Care Assessment Manual, My Aged Care system, IAT, delegation and submission processes.
Educate and support clients to understand the assessment process, eligibility, aged care services and appropriate referrals based on their care needs.
Liaising with referral sources and community support organisations to assist our clients to obtain the home supports they require.
Ensuring assessments are competed accurately and within required timeframes, including comprehensive documentation, data recording, eligibility determination and support planning.
About you
You are a proactive employee that is able to work independently, managing a high-volume caseload while respecting clients’ choices, preferences, and dignity. You can demonstrate strong assessment, communication and report writing skills, with knowledge of wellness and reablement approaches, and the Aged Care Act.
You have:
A current AHPRA registration as a registered nurse or allied health professional
Current drivers licence
Reliable vehicle
Experience working in Aged care sector
Ability to obtain an NDIS screening check or Police check
Ability to obtain a Working with Children Check
