NBMLHD Community Mental Health offers a range of specialised services for children, young people, families and older people, through our community-based services and health centers. Our multidisciplinary health professionals and case managers take a collaborative approach to mental health care by linking with their patients to work together and consult within an environment appropriate to the person's needs.
Make a difference in the lives of young people and help shape their mental health journey. We are seeking a passionate and experienced Clinical Nurse Consultant to join the Youth Mental Health Initiative (YMHI), where you will play a pivotal role in supporting young people aged 12 to 24 with complex mental health needs. Working in partnership with headspace and broader mental health services, you will provide specialist clinical expertise, helping young people access the right care at the right time while promoting positive mental health outcomes.
In this rewarding role, you will deliver advanced clinical assessments, care coordination, and short-term interventions within a stepped model of care. As a trusted nursing leader, you will provide expert consultation and support to headspace clinicians, strengthening their capacity to respond to complex presentations and enhancing the quality of care delivered to young people. You will also play a key role in facilitating seamless pathways across Child, Youth and Family Mental Health Services, headspace sites, and wider mental health services, ensuring continuity of care and a coordinated approach to treatment and recovery.
