About The Role
Champion Functional Independence: Partner with a multidisciplinary team and, when needed, clients' families/carers to assess and resolve functional and mobility challenges.Apply Purposeful Assessment Tools: Conduct functional assessments focused on clients’ specific goals and diagnoses to guide meaningful interventions.Recommend Life-Enhancing Equipment: Evaluate and prescribe mobility aids and coordinate with external suppliers to secure the best-fit solutions for clients.Design and Deliver Targeted Therapy: Create, execute, and assess personalized restorative or maintenance exercise programs, complete with clear, written recommendations to guide ongoing care.Lead Prevention Education: Collaborate with the interprofessional team to educate clients and carers on fall prevention and strategies to slow functional decline.Drive Oedema Management in Home Care: Lead and support the Home Care team in proactively preventing and managing chronic oedema.Connect Clients to Reablement Supports: Inform clients and families about disability and reablement options, and facilitate referrals to further services as needed.

Qualifications in PhysiotherapyRegistered with AHPRACurrent NSW driver license and use of registered motor vehicleCurrent NDIS Worker Check (NDISWC)
