Make a Difference Through Social Work

Temporary Full-Time to Join Logan Hospital’s Social Work team and support patients who are homeless or at risk of homelessness. Deliver meaningful, person-centred interventions across the Emergency Department and inpatient wards while working closely with multidisciplinary teams.

Your new role:

As a Social Worker – Accommodation Support, you will provide high-quality Social Work services to patients experiencing homelessness or housing instability across Logan Hospital. You'll undertake clinical assessment, therapeutic intervention, patient education and discharge planning while working collaboratively with patients, families, healthcare professionals and external agencies.

Key Responsibilities

• Provide Social Work assessment, intervention and discharge planning for patients experiencing homelessness or housing risk.
• Manage a diverse caseload across the Emergency Department and inpatient wards.
• Work collaboratively with multidisciplinary teams, patients, families and external agencies.
• Contribute to student education, quality improvement, research and service development.

About you:

You are a compassionate and capable Social Worker who can build strong relationships with people experiencing complex social circumstances. You'll bring sound clinical judgement, effective communication and a commitment to delivering evidence-based, person-centred care.

Skills & Experience

• Tertiary qualification in Social Work and eligibility for AASW membership.
• Ability to deliver high-quality, evidence-based Social Work assessment and intervention.
• Strong communication, collaboration, negotiation and interpersonal skills.
• Commitment to professional development, quality improvement and patient-centred care.

Logan Hospital is the major health centre for one of the fastest growing areas in Queensland. We offer a range of services, including medical, rehabilitation, maternity, surgical and paediatric care.

We offer our staff and students extensive support and training in most medical specialties. As a teaching hospital, we help train future health care professionals and participate in research through strong partnerships with Queensland's leading medical schools, universities, and TAFE Queensland.

Our services will continue to grow as we build bigger and better facilities to support the community. By 2027, we'll have more beds and 5 more birthing suites as part of the Logan Hospital expansion.
