Community Nurse

The Community Nurse provides comprehensive clinical assessment, care planning, health education, care coordination and nursing interventions on referral from a Care Partner.

The Care Partner retains primary responsibility for ongoing client relationship management.  Working as part of a multidisciplinary team, the Community Nurse supports clientsl to maintain their health, independence, safety and quality of life through evidence-based nursing care, early identification and management of clinical risks, and delivery of person-centred, culturally responsive and trauma-informed practice.

The role contributes to clinical governance, quality improvement and service coordination activities and provides clinical advice to support effective care planning and service delivery across community aged care and disability services.

Given Jewish Care's focus on supporting older members of the Jewish community, including Holocaust Survivors, the Community Nurse demonstrates an understanding of the impact of trauma across the lifespan and applies trauma-informed principles to all interactions and clinical interventions.
