We are looking for an experienced Registered Nurse – Grade 3A to join our Nursing and Medical Services team on a permanent part-time basis (4 days per week).

About the role

Reporting to the Clinical Nurse Consultant, you will work as part of an interdisciplinary team to provide high-quality palliative care to clients, families and carers in the community.

You will:

• Provide comprehensive, holistic nursing assessments and develop individualised care plans.

• Deliver, evaluate and coordinate high-quality palliative nursing care in collaboration with clients, families, carers and other health professionals.

• Build trusting relationships with clients and their families, ensuring their needs, choices and wishes remain at the centre of care.

• Provide clinical advice, education and advocacy to clients, families and carers.

• Work collaboratively with GPs, health professionals and other members of the interdisciplinary team.

• Make advanced clinical decisions and respond appropriately to changing client needs.

• Contribute to continuous improvement, evidence-based practice, research and quality activities.

• Support and mentor colleagues and student placements as required.

• Maintain accurate client records and effectively manage your workload with a high degree of autonomy.

• Provide culturally appropriate, respectful and inclusive care.

About you

You are an experienced Registered Nurse who brings strong clinical capability, compassion and a genuine commitment to palliative care.

Desirable requirements:

• At least 12 months' community-based palliative care experience; or a minimum of four years' post-registration nursing experience

• A postgraduate qualification (or currently working towards one) in palliative care, oncology, community nursing or a related field

• Well-developed interpersonal and written communication skills.

• The ability to work independently and manage your workload effectively.

• Strong organisational and time management skills.

Essential Requirements:

• Current AHPRA registration as a Registered Nurse

• A Certificate, Diploma or Degree in General Nursing that meets AHPRA registration requirements.

• Current National Police Check

• Current Working With Children Check

• Working rights in Australia

• A current Victorian driver's licence
