NDIS Support Coordinator

We are seeking an experienced NDIS Support Coordinator to help participants understand and implement their NDIS plans, Key duties include attending NDIS plan and review meetings, meeting regularly with participants, coordinating care-team meetings, implementing NDIS plans and processes, monitoring funding and supports, liaising with providers and preparing progress, implementation and plan-review reports.

Previous NDIS Support Coordination experience is mandatory.

Must Have

• Experience attending NDIS planning and plan-review meetings
• Experience conducting participant and care-team meetings
• Strong understanding of NDIS processes and plan implementation
• Experience preparing implementation, progress and plan-review reports

Applicants without direct NDIS Support Coordination experience will not be considered.

Pay: $76,000.00 – $95,000.00 per year
