Your new role

As a Client Assessment and Support Planner, you’ll work directly with younger people with disability or health conditions and their carers, helping them access the support they need to live as independently as possible as part of HACC PYP.

You’ll conduct holistic, strengths-based assessments, often in clients’ homes, and work with each person to understand their goals and develop practical support plans. You’ll research available services, coordinate referrals and connect clients with community, health and government-funded supports that best meet their needs.

You’ll manage a varied caseload and continue to review supports as people’s circumstances change. Working both independently in the community and collaboratively with a small team, you’ll build relationships with clients, carers, service providers and other agencies to achieve positive, sustainable outcomes.

About HACC PYP

Home and Community Care Program for Younger People (HACC PYP) is for people aged under 65 and Aboriginal people aged under 50 who need assistance with daily activities, including personal care, dressing, preparing meals, house cleaning, property maintenance, community access and using public transport. The program has been designed to support people with disabilities, a medical or health condition, and their respective carers to live as independently as possible in their own homes.

Your skills and experience

You’ll be someone who can build rapport with people from a wide range of backgrounds, understand the bigger picture of their needs and connect them with practical supports that make a difference.

Ideally, you’ll bring:

• Experience conducting holistic, strengths-based assessments, ideally within people’s homes or community settings, and developing practical support or care plans.
• Experience working with a diverse range of clients, including younger people with disability, health conditions and/or their carers.
• The ability to research and navigate community and government services, identify appropriate supports and make effective referrals that build a person’s capacity and independence.
• Strong client engagement and communication skills, with the ability to work collaboratively with clients, carers, service providers and other agencies.
• Sound judgement and the confidence to work independently in the community while contributing to a small, collaborative team.
• Good documentation and administrative skills, including maintaining accurate assessments, case notes and client records.
• A relevant tertiary qualification in Health or Community Services, such as Social Work, Psychology or Occupational Therapy, or relevant demonstrated experience.
• A current Australian driver’s licence.
