About the role
As a Client Assessment and Support Planner, you’ll work directly with younger people with disability or health conditions and their carers, helping them access the support they need to live as independently as possible as part of HACC PYP.
You’ll conduct holistic, strengths-based assessments, often in clients’ homes, and work with each person to understand their goals and develop practical support plans. You’ll research available services, coordinate referrals and connect clients with community, health and government-funded supports that best meet their needs.
You’ll manage a varied caseload and continue to review supports as people’s circumstances change. Working both independently in the community and collaboratively with a small team, you’ll build relationships with clients, carers, service providers and other agencies to achieve positive, sustainable outcomes.
Home and Community Care Program for Younger People (HACC PYP) is for people aged under 65 and Aboriginal people aged under 50 who need assistance with daily activities, including personal care, dressing, preparing meals, house cleaning, property maintenance, community access and using public transport. The program has been designed to support people with disabilities, a medical or health condition, and their respective carers to live as independently as possible in their own homes.
