Join the Transitional Aged Care Program (TACP) and make a meaningful difference in the lives of older adults as they regain independence and work towards their personal goals following a hospital stay. TACP is a unique, client‑centred reablement program that supports older people to maximise their health, wellbeing and quality of life within the community. As an Allied Health Assistant, you will play an important role in delivering therapy and care within clients’ homes and residential aged care settings, supporting interventions such as home exercise programs, self‑care retraining, community access and transport to appointments. Working across our community teams in Bankstown and Fairfield, or within our Residential TACP program based in Carramar, you will be part of a multidisciplinary team committed to achieving the best possible outcomes for older people and their families.

You are an enthusiastic and dedicated Allied Health Assistant who enjoys working with older people and is passionate about supporting them to achieve their goals and maintain independence. You have strong communication and problem‑solving skills and are comfortable working both independently and as part of a multidisciplinary team. You are committed to providing high‑quality care under the guidance of Allied Health Professionals and Team Leaders and are confident supporting clients and their families in a respectful and person‑centred manner. You take pride in contributing to effective service delivery through both clinical and administrative support activities and are motivated to make a positive impact within a rewarding community and aged care setting.

Certificate IV in Allied Health Assistance or relevant qualification or Minimum 12 months full-time equivalent relevant experience with commitment to undertake the Certificate IV in AHA within 12 month periodCan you describe a time when you were delegated a task by a supervising Allied Health Professional that required you to manage competing priorities? How did you ensure you stayed within your scope of practice, met deadlines, and sought assistance when needed?In your previous roles, how have you adapted your communication style to effectively interact with both clinical staff and patients? Can you also share how you’ve applied Work Health and Safety principles in a healthcare setting?Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted
