We’re looking for a compassionate and motivated Registered or Enrolled Nurse to join our supportive team at Wyoming Medical Centre. In this role, you’ll work closely with GPs and other healthcare professionals to help patients manage chronic conditions, improve their wellbeing and take a more active role in their health.

Part-time and full-time opportunities are available.

The Role

Your responsibilities will include:

• Preparing care plans and coordinating multidisciplinary care arrangements
• Supporting patients to manage chronic conditions and achieve their health goals
• Conducting health assessments and promoting preventative healthcare
• Coaching patients to make sustainable health and lifestyle changes
• Collaborating with GPs, allied health professionals and the broader practice team
• Coordinating referrals, follow-up appointments and ongoing patient reviews
• Completing accurate clinical documentation and associated administrative tasks

About You

You will bring:

• Current AHPRA registration as a Registered Nurse or Enrolled Nurse
• Previous experience in chronic condition management, primary care or general practice—preferred
• Strong communication, organisation and patient-engagement skills
• A collaborative approach and confidence working within a multidisciplinary team
• A friendly, proactive and patient-focused attitude
• A genuine passion for preventative healthcare and improving patient outcomes
