Refugee Health Nurse

About the Role

The Multicultural Access Program Nurse will support newly arrived refugees and migrants across the Ipswich region to access and navigate the Australian healthcare system. Based at the SSI Goodna Office, with regular outreach across the Ipswich region, you will conduct health needs assessments, connect clients with GPs, allied health and specialist services, and support coordinated ongoing healthcare.

You will play a key role in improving health outcomes, particularly for people living with chronic health conditions, while promoting access to culturally responsive and appropriate mainstream health services. The role will also involve building strong relationships with health stakeholders and supporting healthcare providers to better respond to the needs of newly arrived communities.

This position is a Full-time max term contract (Maternity Cover) until 20th August working hybrid from our Goodna Office. 

Key Responsibilities

• Conduct health and social needs assessments and develop individual care plans.
• Coordinate referrals, health appointments and required health screenings.
• Support immunisations, pathology, TB screening and childhood development checks.
• Conduct 3- and 6-month reviews and exit interviews to monitor client outcomes.
• Provide health education, advice and support across relevant SSI programs.
• Build strong partnerships and referral pathways with health networks across West Moreton.
• Work with GPs to strengthen culturally responsive care and improve client engagement.
• Advocate for clients’ health needs and rights while supporting effective access to services.
• Monitor client demand and GP capacity and implement support strategies where required.
• Prepare and maintain monthly, quarterly and annual reports and statistics.
• Other duties as directed from time to time within your skills range and capacity.

Essential Criteria

• Current AHPRA registration as a Registered Nurse and a Bachelor of Nursing or equivalent qualification.
• Demonstrated experience in primary healthcare and General Practice, including conducting health assessments with a chronic disease focus.
• Knowledge and understanding of the health needs of refugees, asylum seekers and culturally and linguistically diverse (CALD) communities, including effective use of interpreters.
• Demonstrated experience in coordinated case management, including referrals and connecting clients with external health and support services.
• Experience working with CALD clients, with the ability to provide culturally responsive and person-centred care.
• Strong understanding of social and health needs and the ability to work collaboratively with multidisciplinary teams and community services.
