About the role
CareWA Services is seeking a compassionate and dedicated Case Manager - NDIS Sector to join our growing team in Myaree, WA. This is a full-time position that plays a vital role in supporting individuals with disabilities to achieve their goals and maximise their independence through the National Disability Insurance Scheme (NDIS). As a Case Manager, you will be at the forefront of delivering person-centred support, building meaningful relationships with participants, and coordinating comprehensive care plans that enable positive life outcomes.
Key responsibilitiesConduct comprehensive assessments of participant needs, goals, and aspirations to develop individualised support plans aligned with NDIS objectivesCoordinate and manage support services, ensuring participants receive appropriate and timely interventionsEstablish and maintain strong, trusting relationships with participants and their families or carersMonitor participant progress against agreed goals and adjust support strategies as requiredLiaise with external service providers, medical professionals, and community organisations to ensure holistic support deliveryPrepare and maintain detailed case notes and documentation in compliance with NDIS requirements and organisational policiesAdvocate for participant rights and ensure their voice is central to all decision-making processesIdentify and address barriers to participation, inclusion, and access to community resourcesParticipate in team meetings and contribute to continuous service improvement initiativesSupport participants to build their capacity and independence over time
What we're looking for
Tertiary qualification in Social Work, Disability Services, Aged Care, Nursing, Psychology, or a related discipline (Not essential but preferred)Demonstrated experience working in the disability sector, NDIS environment, or community servicesStrong understanding of the NDIS framework, participant planning processes, and support coordination principlesExcellent interpersonal and communication skills with the ability to engage sensitively with participants from diverse backgroundsProven ability to develop and maintain professional relationships with participants, families, and external stakeholdersStrong organisational and time management skills with the ability to manage multiple cases effectivelyProficiency in using IT systems and electronic case management softwareCurrent Working with Children Check (or willingness to obtain) and a valid Australian driver's licenceDemonstrated commitment to person-centred practice and social justice principlesAbility to work both independently and collaboratively within a team environmentPreferred: Experience with outcome measurement and data collection in disability support services
