About the role
We are seeking a dedicated Community Registered Nurse to join the team at STAR Community Services in Cleveland, QLD. This is a part-time position that offers the flexibility to balance professional nursing responsibilities with other commitments. As a Community Registered Nurse, you will play a vital role in delivering high-quality nursing care and support to vulnerable members of our community, including elderly and disabled individuals. This position is essential to our mission of providing compassionate, person-centred care that enables individuals to maintain their independence and quality of life within their own communities.
Key responsibilities
Provide direct nursing care and clinical assessment to elderly and disabled clients in community settings
Conduct comprehensive health assessments and develop individualised care plans in collaboration with clients and their support networks
Administer medications and undertake clinical procedures in accordance with nursing best practice and regulatory requirements
Monitor client health status and identify any changes that may require medical intervention or escalation
Provide health education and support to clients and their families or carers to promote self-management and wellbeing
Maintain accurate, timely and confidential client records and documentation
Liaise with multidisciplinary teams, including general practitioners, allied health professionals and social workers, to ensure coordinated care
Support clients with wound care, continence management, catheter care and other clinical nursing interventions
Promote and advocate for client rights, dignity and person-centred care principles
Contribute to quality improvement and evidence-based practice within the community care setting
What we're looking for
Current Australian nursing registration as a Registered Nurse with AHPRA and a valid Nursing and Midwifery Board of Australia (NMBA) registration
Current First Aid and Cardiopulmonary Resuscitation (CPR) certification
Demonstrated experience in community nursing, aged care, disability support or a related healthcare setting
Strong clinical knowledge and competence in assessment, care planning and nursing interventions appropriate to elderly and disabled populations
Excellent communication and interpersonal skills with the ability to build rapport with clients, families and multidisciplinary teams
Ability to work independently whilst maintaining professional accountability and seeking support when needed
Commitment to person-centred care principles and respect for client autonomy, dignity and confidentiality
Physical and mental capability to perform the demands of community nursing, including home visiting and manual handling
A valid driver's licence and access to reliable transport for home visits
Working with Children Check and Police Clearance (or willingness to obtain these if not currently held)
Preferred experience with electronic health records, care coordination systems and community nursing documentation
