Turn participant goals and NDIS funding into meaningful outcomes
Location: Moonah, Hobart TAS

Salary: SCHADS Award Level 4 plus Salary Packaging
At Langford Support Services, we believe people with disability should have genuine choice, control and a voice in the supports they receive.
We are looking for a Participant Planner who can bring together people, planning and practical service delivery, someone who understands the NDIS, builds strong relationships and can turn participant goals and funding into support arrangements that work in real life.
This is more than a paperwork role.
You will work directly with participants, families, nominees, advocates and support networks, while partnering closely with our operational teams to ensure each participant's goals, funding and support arrangements are clearly understood and effectively delivered.

The OpportunityAs our Participant Planner, you will support participants throughout their journey with Langford from establishing services through to ongoing planning, reviews and changes in circumstances.
You will act as an important link between participants, their NDIS plans and the services Langford delivers.
Your role will include:building trusted relationships with participants, families, nominees, advocates and other stakeholders
developing and maintaining Individual Support Plans and Service Agreements
translating participant goals into practical and measurable support arrangements
monitoring participant funding utilisation and identifying emerging funding or service delivery risks
supporting NDIS reassessments, changes of circumstances and evidence gathering
explaining services, funding arrangements and participant responsibilities clearly and respectfully
working collaboratively with Program Managers, Team Leaders, Rostering, Finance and Quality teams
ensuring participant plans and support arrangements are understood and implemented in practice
maintaining accurate, current and audit-ready participant records
supporting participant choice, control, independence and supported decision-making
identifying barriers or gaps affecting participant outcomes and helping find practical solutions
contributing ideas that improve participant experiences, systems and service delivery.
You understand that good participant planning starts with listening.
You are comfortable working with NDIS plans, funding and detailed documentation, but equally confident sitting down with a participant or family and having a genuine conversation about their goals, needs and support arrangements.
You will bring:
relevant experience in disability, NDIS, community services, health, aged care or human services
a sound working knowledge of NDIS plans and funding arrangements
strong relationship-building and communication skills
clear and professional written communication skills
strong organisation and attention to detail
confidence managing multiple participant matters and competing priorities
the ability to identify funding, service delivery or documentation risks and raise them appropriately
sound judgement, accountability and a commitment to participant rights, dignity, choice and inclusion
confidence working collaboratively across different teams and stakeholders.
Experience in support coordination, participant planning, case management, service coordination or a similar role would translate well to this position.
You do not need to have previously held the title Participant Planner , what matters is your ability to connect participant goals, NDIS funding and practical service delivery.
You will need:
a qualification in Disability, Community Services, Human Services, Health or a related discipline, or equivalent demonstrated experience
relevant experience within disability, community services, health, aged care or human services
working knowledge of the NDIS, participant plans and funding arrangements
current Registration to Work with Vulnerable People with NDIS endorsement, or the ability to obtain it
a current National Police Check
a current driver's licence.
Experience preparing Service Agreements or Individual Support Plans, supporting NDIS reassessments or changes of circumstances, monitoring participant funding or working with NDIS quality and safeguarding requirements will be highly regarded.
