NDIS Support Providers Pty Ltd is a growing and client-focused organisation dedicated to assisting participants in achieving their goals and accessing the supports they need. Due to our expanding client base, we are seeking an experienced and motivated Support Coordinator to join our team on a full-time basis.

About the Role

As a Support Coordinator, you will work closely with NDIS participants, their families, carers, and service providers to help participants understand and implement their NDIS plans. You will support participants to build capacity, connect with services, and achieve positive outcomes in line with their goals.

Key Responsibilities

·       Assist participants to understand and implement their NDIS plans.

·       Coordinate and connect participants with appropriate supports and services.

·       Develop and maintain effective relationships with participants, families, guardians, and service providers.

·       Monitor participant progress and outcomes.

·       Maintain accurate case notes, records, and reports.

·       Advocate for participants and support them to exercise choice and control.

·       Ensure compliance with NDIS Practice Standards and organisational policies.
