Right Quality Care Pty Ltd is seeking a Registered Nurse – NDIS Assessments, Care Plans & Intake to join our growing team in Sydney. This is a casual position that offers flexibility whilst making a meaningful impact in the lives of individuals with disabilities. As a Registered Nurse, you will play a crucial role in conducting NDIS assessments, developing comprehensive care plans, and managing intake processes for our clients. This position is essential to ensuring that participants receive individualised, high-quality support tailored to their specific needs and goals within the National Disability Insurance Scheme framework.
Key responsibilities
Conduct thorough NDIS assessments and evaluations to determine participant needs, goals, and support requirements
Develop and maintain comprehensive, individualised care plans based on assessment findings and participant aspirations
Manage intake procedures for new NDIS participants, ensuring all documentation is completed accurately and in compliance with scheme requirements
Collaborate with multidisciplinary teams, participants, and their families to gather relevant information and ensure holistic care planning
Provide clinical nursing expertise and recommendations to support participant wellbeing and independence
Monitor and review care plans regularly, updating them in response to changing participant circumstances or needs
Liaise with external healthcare providers and allied health professionals to coordinate comprehensive support
Ensure all assessments and documentation comply with NDIS legislation, policies, and Right Quality Care standards
Maintain accurate and confidential records using electronic systems and documentation platforms
What we're looking for
Current registration with the Nursing and Midwifery Board of Australia (NMBA) as a Registered Nurse
Demonstrated experience working within the NDIS framework, including assessments and care planning
Strong knowledge of aged care and disability support services, including understanding of participant-centred approaches
Excellent clinical assessment skills and the ability to identify complex support needs
Proficiency in electronic health records and data management systems
Outstanding written and verbal communication skills, with the ability to explain complex information clearly to participants and their families
Strong organisational skills with the ability to manage multiple assessments and intake processes simultaneously
Demonstrated commitment to person-centred care and respecting participant autonomy and choice
Ability to work independently and as part of a collaborative team
Current Working with Children Check and/or other relevant security clearances desirable
