This Position requires an Individual who has:Experience providing NDIS Support Coordination/Recovery Coaching Services or Similar Role Support participants to understand and implement their NDIS plansAssist participants in building capacity, independence, and decision-making skillsCoordinate and connect participants with appropriate service providersMonitor supports and ensure services align with participant goals and fundingA Solid understanding of the NDIS and its Importance to People with a Disability and their Support NetworksDemonstrated experience with Facilitating Multi-Disciplinary Coordinated Planning and Develop Collaborative Relationships and Networks between Service ProvidersMaintaining an Up-To-Date knowledge of the NDIS Systems and PracticesEngaging with NDIA Staff and LAC’sExperience with Providing RIDS Monthly reporting Liaising with the Participant, Family or Representative on Supporting the Participant with Achieving their GoalsPreparing for Plan Reviews and Attending with the Participant, Family or RepresentativeMaintain accurate and compliant documentation in line with NDIS requirements
