Registered Nurse – Telehealth Care Coordinator – St Leonards (Office Based)

Your New Role

As a member of our Telehealth Care Coordinator team, you will play an important role in delivering high-quality, patient-centred care through Vitalis Health & Home Care’s telehealth services. The role supports patients across the pre-operative and post-operative continuum, providing assessment, education, and guidance to ensure safe and effective recovery.

 What We’re Looking For:

• A Seasoned Clinician: A Registered Nurse with AHPRA registration and a strong background (5+ years)
• An Experienced Practitioner: Specialty experience in General Surgery, Oncology, Gynaecology, Cardiac, Orthopaedics, or Urology (recent or current). 
• A Strong Communicator: Able to provide clear education, coaching, and guidance to patients and carers, while liaising effectively with multidisciplinary teams and external healthcare providers.
• Digital Health Capability: Experience with digital health systems & electronic documentation.
• A Compassionate collaborator: A team player who thrives in patient-centred care
