Hybrid Support Coordinator/Plan Manager

About the role

You will work closely with clients to provide practical, person-centred Support Coordination while also working alongside the Plan Management function to ensure supports are appropriately claimed, recorded and managed against participant plans.

Key responsibilities

• Providing Support Coordination to a small caseload of NDIS participants
• Maintaining regular phone, email and in-person contact with clients
• Helping participants understand and use their NDIS plans effectively
• Connecting participants with appropriate services and providers
• Coordinating supports and communicating with families, providers and other stakeholders
• Monitoring participant funding and identifying potential issues early
• Working closely with Plan Management to ensure services are appropriately coded and claimed against participant funding
• Understanding NDIS support categories, line items and relevant claiming processes
• Maintaining accurate client records and case notes
• Supporting participants with plan implementation, service agreements and provider coordination

About you

• Previous experience working in Support Coordination
• A strong understanding of NDIS processes and participant plans
• Good knowledge of NDIS support categories, line items and support item codes used for claiming/reimbursement
• An understanding of how participant funding is allocated and monitored
• Experience communicating with participants, families, providers and other professionals
• Excellent organisational and time-management skills
• Strong written and verbal communication
• Confidence working autonomously from home
• Good computer and administration skills
• A professional, practical and solutions-focused approach
