About the role
About the role
In this newly created role, the Project Officer will support the development, implementation and delivery of the Palliative Medicines in Community Pharmacies (PMCP) program. The position aims to strengthen the role, function and scope of community pharmacies in palliative care, supporting better access to palliative medicines and pharmacy services for Canberrans receiving palliative care at home. The role works collaboratively with other sector stakeholders in the ACT to co-design program and evaluation framework, and coordinate program deliverables, including with palliative prescribers, health care service providers and ACT Health and Community Service Directorate (HCSD).
About the team
The Project Officer will be part of the Palliative Care Planning Team and report to Palliative Care Planning Manager. The team oversees the implementation of the Greater Choice for At Home Palliative Care (GCfAHPC) Program across the ACT by collecting and generating data to monitor performance, evaluate services and contribute to continuous improvement. The team sits within the broader Health System Integration Team that is responsible for coordinating projects and program areas centred around improvement and integration of the health system to actively respond to the needs of the ACT community.
See yourself in our team
You will be a dynamic project management professional who shows intuition, sound judgement and ability to balance independent work with team collaboration. You will bring transferable skills in stakeholder engagement, co-design and program evaluation to effectively liaise with community pharmacies, medicine prescribers, policy makers and consumers, helping to improve capability of the service providers and access for the end consumers at the same time. This includes supporting PMCP pharmacy recruitment, establishing communities of practice, and collaborating with specialist palliative care and primary care providers to develop an ACT Palliative Care Core Medicines List for permanent stockholding. The role also facilitates targeted training and education, and implements quality improvement initiatives to enhance access to palliative care medicines at home.
Your experience in managing cross-sector health programs within the primary, community or aged care settings, combined with familiarity of the ACT palliative care systems, stakeholders and service delivery models will allow you to contribute meaningfully to the program objectives. Your qualifications and registration as a pharmacist will be highly desirable, but not essential.
