About the role
Community Support Worker – Western Sydney, St George & Eastern Suburbs
Quality Health Care (QHC) is seeking compassionate and reliable Community Support Workers to join our growing Home Care Services team.
We currently have opportunities available across Penrith, Hurstville, Rockdale and Randwick, supporting clients to remain independent in their own homes and communities.
Our clients have a diverse range of support needs, including physical disabilities, mental health conditions, psychosocial support requirements, age-related needs, and assistance with daily living activities. We are looking for support workers who are committed to providing high-quality, person-centred care and building positive relationships with clients.
Your Role:
· Providing person-centred supports in clients' homes and the community
· Supporting people living with mental health and psychosocial support needs
· Assisting with personal care and daily living activities
· Supporting clients with mobility, physical health and disability-related needs
· Community access including appointments, shopping, social outings and recreational activities
· Domestic assistance including cleaning, laundry and meal preparation
· Transporting clients to appointments and community activities
· Medication prompting and wellbeing monitoring
· Encouraging independence, choice and goal achievement
· Maintaining accurate documentation and progress notes
What We're Looking For:
· Experience working in disability, home care, community services, aged care or mental health support
· Confident supporting a variety of client needs and support requirements
· Strong communication and relationship-building skills
· Ability to work independently and adapt to different client environments
· Reliable, professional and person-centred approach
· Current Driver Licence (Essential)
· Own reliable and insured vehicle (Essential)
· Current First Aid & CPR Certificate (Essential)
· Current NDIS Worker Screening Check (or willingness to obtain)
· Relevant qualifications highly regarded
