We're looking for a compassionate Registered Nurse | Clinical Care Partner to join our growing Support at Home team. While this role is ideally full-time, we're happy to discuss flexible part-time arrangements for the right candidate.
Based from our Chermside office, you'll enjoy a flexible hybrid work model, working across home, office and the community while supporting older people throughout North Brisbane and Moreton Bay.
Working alongside an experienced multidisciplinary team, you'll combine your clinical expertise with care coordination to help clients remain safe, independent and connected in the place they most want to be – home. Whether your background is community nursing, residential aged care, general practice or another clinical setting, this role offers greater autonomy, flexibility and variety while making a genuine difference every day.

About you
You'll be a Registered Nurse who enjoys building relationships just as much as delivering excellent clinical care.
You'll also bring:
Experience in community nursing, general practice or residential aged care will be highly regarded.
Experience managing aged care budgets and undertaking client risk assessments will also be highly regarded.
