Your Care Co is looking for an experienced Team Leader – Workforce & Service Delivery to join our growing team in Geelong.

This is an opportunity for someone who can combine strong NDIS knowledge, people leadership, workforce coordination and participant-focused service delivery to make a real difference.

About the role

You will play a key role in coordinating and leading the day-to-day delivery of participant services while supporting and developing a skilled, engaged and compliant workforce.

You will work closely with participants, families, Support Coordinators, brokerage partners, allied health professionals and community organisations to ensure high-quality, consistent and person-centred supports.

We are looking for someone who:

• Has extensive experience within the NDIS and disability sector
• Has strong knowledge of participant care and person-centred service delivery
• Has established connections and networks within the disability, aged care and community sectors
• Can identify and develop referral and brokerage opportunities
• Has experience leading, mentoring, coaching and supporting frontline teams
• Is confident managing workforce planning, rostering and service coordination
• Has experience recruiting, onboarding and retaining quality support workers
• Can build strong, trusted relationships with participants, families and stakeholders
• Understands NDIS compliance, Practice Standards and quality requirements
• Is a confident communicator who can problem-solve, make decisions and lead from the front

Pay: $80,000.00 – $110,000.00 per year
