Social and Emotional Wellbeing Team Care Coordinator

Pius X Aboriginal Corporation is seeking an Aboriginal Mental Health Counsellor / Care Coordinator to join our progressive, culturally grounded Social & Emotional Wellbeing team. This is an Identified Aboriginal Position, open only to Aboriginal applicants under Section 14D of the Anti‑Discrimination Act 1977 (NSW).

About the role

You will deliver culturally safe, trauma‑informed counselling, mental health assessments, brief interventions, and ongoing psychosocial support. You will help clients navigate PHN stepped‑care pathways, attend appointments, access services, and receive timely follow‑up, outreach, and welfare checks.

You will also contribute to cultural leadership, clinical governance, and continuous improvement across the SEWB team.

Key responsibilities

• Provide culturally safe, trauma‑informed counselling, brief interventions, and therapeutic support.

• Complete mental health assessments, suicide risk assessments, care plans, and psychosocial reviews.

• Deliver care coordination, referrals, linkage, and follow‑up across PHN stepped‑care pathways.

• Conduct outreach, home visits, welfare checks, and follow‑up contacts in line with WHS protocols.

• Maintain accurate case notes, PRMs/PROMS, PMHC MDS reporting, and documentation.

• Participate in multi‑agency case conferencing with GPs, clinicians, AMS staff, and partner organisations.

• Provide cultural mentoring and guidance to Peer Navigators and SEWB team members.

• Uphold cultural safety, trauma‑informed practice, privacy, and professional boundaries.

What you’ll bring

• Aboriginality (Identified Position – Section 14D exemption applies).

• Qualifications in Counselling, Social Work, Psychology, Mental Health, or related field.

• Experience providing culturally safe, trauma‑informed counselling to Aboriginal and Torres Strait Islander clients.

• Strong skills in mental health assessment, suicide risk assessment, safety planning, and brief interventions.

• Experience in care coordination, case management, referrals, and multi‑agency collaboration.

• Ability to conduct outreach, home visits, and welfare checks safely.

• Excellent communication, rapport‑building, and de‑escalation skills.

• Strong documentation skills, including case notes and PMHC MDS reporting.

• Commitment to cultural supervision, clinical supervision, and continuous improvement.

• Current First Aid, WWCC, Police Check, Driver’s Licence, and Prohibition Notice Declaration.
