Program Manager, Health, Chronic Disease & Ageing

Amity Health is seeking an experienced, operational and values-driven Program Manager, Health, Chronic Disease & Ageing to lead a diverse portfolio of programs supporting people living with primary health, chronic health conditions, needs associated with ageing, and complex care requirements across the Great Southern, Northern & Southern Wheatbelt, and Southern Coastal Goldfields regions. 

Reporting to the Chief Operating Officer, this pivotal leadership role offers the opportunity to influence service delivery, support multidisciplinary teams, strengthen community partnerships, and drive positive outcomes for priority populations throughout regional Western Australia.

The Opportunity 

This is an exciting opportunity to lead Amity Health’s Primary Health, Chronic Disease & Ageing service stream and contribute to the delivery of integrated, person-centred and high-quality health services. 

You will provide operational leadership across a portfolio focused on early intervention, chronic disease management, care coordination, healthy ageing, Aboriginal health outcomes, and improving access to community and specialist services. 

The programs within this portfolio include: 

• Integrated Chronic Disease Care (ICDC) 
• Integrated Team Care (ITC) 
• care finder 
• Strengthening Aboriginal Health 
• Visiting Specialists and Program Support Services 

What You’ll Lead 

In this role, you will oversee the operational performance, governance, risk and compliance and service development of Amity Health’s Health, Primary Health, Chronic Disease & Ageing programs. 

Working collaboratively with internal teams, community partners, external healthcare providers and funding bodies, you will ensure services remain accessible, equitable, culturally safe and responsive to community needs. 

You will have the opportunity to: 

• Shape and lead a newly established leadership portfolio. 
• Support multidisciplinary teams to deliver high-quality, person-centred and culturally safe services. 
• Strengthen integration across primary health care, allied health, aged care and community services. 
• Build and maintain strategic partnerships with community organisations, healthcare providers, government agencies and funding bodies. 
• Drive innovative, prevention-focused approaches that improve health and wellbeing outcomes. 
• Oversee program performance, governance, risk management, budgets and funding obligations. 
• Build organisational capability through coaching, mentoring and leadership development. 
• Champion equitable access to services and address health inequities experienced by vulnerable individuals with complex needs. 

This is a full-time position based in Albany, with regular travel required across regional and rural communities. 

About You 

You are a confident, compassionate and experienced leader with a strong understanding of integrated health and community services. You are passionate about improving outcomes for regional communities and bring the leadership capability to guide teams, foster partnerships and drive service excellence. 

To be successful in this role, you will demonstrate: 

• Demonstrated leadership experience within primary health, aged care, community services, or a similarly complex human services environment. 
• A minimum of five years’ experience in a frontline clinical role within primary health, community health or a related clinical setting.
• Strong knowledge of integrated, person-centred models of care and their application across chronic disease management, healthy ageing and primary healthcare settings. 
• Demonstrated success leading multidisciplinary teams and driving program delivery, service improvement and operational performance. 
• Experience managing budgets, contracts, funded programs and reporting requirements. 
• Exceptional stakeholder engagement and relationship-management skills, with the ability to build trusted partnerships across health, government and community sectors. 
• A genuine commitment to culturally safe, inclusive and equitable approaches that support diverse communities. 

Essential Requirements 

• Tertiary qualification in Health, Nursing, Allied Health, Public Health, Community Services, Social Work, or a related discipline. 
• Current National Police Clearance. 
• Current Working with Children Check. 
• Current C Class Driver’s Licence. 

Desirable 

• Postgraduate qualifications in Health, Leadership, Management or Human Services. 
• Experience leading teams within trauma-informed practice frameworks. 
• Experience in social impact measurement, quality improvement or program evaluation. 
• Demonstrated leadership experience in governance, risk, compliance and safe service delivery within health, aged care or community service settings. 
• Strong analytical capability, including the ability to interpret data and insights to inform decision-making, improve operational performance and enhance service outcomes.
