About the role
Are you ready to shape a newly created leadership role and make a meaningful impact on integrated health and ageing services across regional Western Australia?
Amity Health is seeking an experienced, operational and values-driven Program Manager, Health, Chronic Disease & Ageing to lead a diverse portfolio of programs supporting people living with primary health, chronic health conditions, needs associated with ageing, and complex care requirements across the Great Southern, Northern & Southern Wheatbelt, and Southern Coastal Goldfields regions.
Reporting to the Chief Operating Officer, this pivotal leadership role offers the opportunity to influence service delivery, support multidisciplinary teams, strengthen community partnerships, and drive positive outcomes for priority populations throughout regional Western Australia.
The Opportunity
This is an exciting opportunity to lead Amity Health’s Primary Health, Chronic Disease & Ageing service stream and contribute to the delivery of integrated, person-centred and high-quality health services.
You will provide operational leadership across a portfolio focused on early intervention, chronic disease management, care coordination, healthy ageing, Aboriginal health outcomes, and improving access to community and specialist services.

What You’ll Lead
In this role, you will oversee the operational performance, governance, risk and compliance and service development of Amity Health’s Health, Primary Health, Chronic Disease & Ageing programs.
Working collaboratively with internal teams, community partners, external healthcare providers and funding bodies, you will ensure services remain accessible, equitable, culturally safe and responsive to community needs.

You are passionate about improving outcomes for regional communities and bring the leadership capability to guide teams, foster partnerships and drive service excellence.
