About the role
Are you passionate about making a meaningful difference in the lives of people with disabilities? Do you have strong organisational skills and the ability to coordinate high-quality support across participants, support workers, families, and other stakeholders?
Open Futures Disability Services is looking for a compassionate, proactive, and experienced Disability Care Coordinator to join our team and help ensure our participants receive safe, person-centred, and high-quality support.
This is a full-time position offering a competitive salary based on experience. As a Disability Care Coordinator, you will play a key role in coordinating participant supports, leading and supporting our disability support workers, and ensuring services are delivered in line with each participant’s needs, goals, and support plans.
The role will support a diverse mix of participants with varying support needs, including participants receiving Supported Independent Living (SIL) services and those with complex support requirements.
