About the role
We are seeking an experienced Clinical Operations Coordinator and Registered Nurse to support the day-to-day clinical and operational management of our growing community-based nursing and support services. This is a hands-on role suited to an experienced RN with a strong background across NDIS and aged care/community care who understands clinical governance, participant safety, quality service delivery and effective workforce coordination. The successful candidate will work closely with Directors, nursing team, support workers, participants, families and external stakeholders to ensure services are safe, responsive, compliant and delivered to a high standard.
Key responsibilities
Oversee the day-to-day clinical operations of NDIS and aged care services
Coordinate onboarding and commencement of new participants and clients
Review referrals and identify clinical, staffing, risk and service requirements
Develop and review individualised Care Plans, Support Plans, risk assessments and clinical documentation
Provide clinical oversight for participants with complex and high-intensity needs
Coordinate services between RNs, ENs, support workers and other members of the care team
Monitor changes in participant needs and ensure appropriate clinical escalation
Support hospital discharge and transition into community-based care
Liaise with hospitals, GPs, allied health professionals, Support Coordinators, families and other providers
Maintain strong knowledge of the NDIS Practice Standards, NDIS Code of Conduct and relevant aged care requirements
About you
Current unrestricted registration as a Registered Nurse with AHPRA
Relevant nursing qualification
Strong NDIS experience
Experience within aged care, community nursing or home-based care
Availability to work a minimum of four days per week
Current unrestricted Australian driver's licence
Current NDIS Worker Screening Check and relevant employment screening requirements
Strong understanding of clinical governance, risk management, incident management and participant safeguarding
Excellent clinical documentation, communication and organisational skills
Mental health and/or psychosocial disability experience is highly desirable
