Coordinate nursing rosters across our Home Care team in Camberwell. 12-month max term, part-time roles with complementary fortnightly rosters — scheduling and community aged/disability care experience valued.

About the Role

We have two exciting opportunities for Direct Care Coordinators to join our Home Care team, based in Camberwell. Working closely with our Nursing Manager and Clinical Care Leads, you'll coordinate, schedule and support our nursing workforce to make sure our clients receive safe, timely and consistent care in their own homes.

This is a hands-on coordination role at the heart of our Home Care operations — you'll manage rosters, respond to gaps and absences, and be a key point of contact between nursing staff, clients and the broader care team.

These are 2 Max Term positions, Part-Time (12 months), working 45 hours per fortnight across a complementary roster:

• Week A: Monday, Tuesday, Wednesday, 8:00am–4:06pm
• Week B: Wednesday, Thursday, Friday, 8:00am–4:06pm

One position will work Week A then Week B; the second will work Week B then Week A, so between the two roles our nursing team is supported every weekday of the fortnight.

What You'll Do

• Coordinate, schedule and validate nursing rosters, aligning coverage with client and service demand
• Proactively identify and resolve gaps in service caused by planned and unplanned absences
• Maintain and adjust rosters to reflect approved changes such as overtime, travel and shift extensions
• Escalate workforce risks that may impact safe, effective care delivery
• Build strong working relationships with nursing staff, Clinical Care Leads and associated external providers
• Support broader operational planning and initiatives across the Home Care team

About You

• Demonstrated experience in rostering or workforce scheduling — ideally within community aged care or community disability care
• Highly organised, with the judgement to work autonomously in a fast-paced environment
• Clear, confident communicator who can juggle competing priorities and keep people informed
• A genuine commitment to supporting older people to live safely and independently at home
• Community aged care or community disability care
• Nursing or healthcare environments
• Medical administration, including managing appointments, bookings or client/patient schedules

You'll also need:

• Satisfactory National Crime Check (renewed every 3 years)
• Full and ongoing right to work in Australia
