The Role

As a Clinical Nurse Specialist – Diabetes, you will provide advanced specialist nursing care, education, and support to people living with diabetes and related endocrinology conditions across the care continuum.

Working within a multidisciplinary team, you will play a key role in improving patient outcomes through education, clinical expertise, and collaboration with internal and external stakeholders.

Key responsibilities include:

• Deliver specialist, client-centred diabetes care, education, and self-management support
• Plan, coordinate and evaluate diabetes education programs and health promotion activities
• Support individuals, families, and carers to manage chronic conditions effectively
• Develop and contribute to clinical guidelines, policies, and quality improvement initiatives
• Provide expert advice and act as a resource for staff and community stakeholders
• Participate in case management for high-risk patients in collaboration with the MDT
• Contribute to staff education, mentoring, and professional development
• Ensure care delivery aligns with evidence-based practice and professional standards

The Team

You will be part of a collaborative multidisciplinary team within Diabetes Services, working closely with nurses, allied health professionals, and medical staff to deliver coordinated, high-quality care.

Details of Appointment

• Permanent part time, day worker position, working 64 hours per fortnight, commencing as soon as you can join our Team.
• Location: Launceston General Hospital

Essential Requirements

• Registered Nurse with the Nursing and Midwifery Board of Australia

Desirable Requirements

• Credentialed Diabetes Educator (CDE) or working towards
• Relevant postgraduate qualification
• Membership with the Australian Diabetes Educators Association (ADEA) or similar

• Strong contemporary knowledge of diabetes management and chronic disease care of greater than 2 years working in a Diabetes Centre
• Highly developed communication and interpersonal skills
• Ability to deliver education programs and support behaviour change
• Experience working collaboratively in multidisciplinary environments
• Commitment to quality improvement, risk management and evidence-based practice
